THE BRIDGE ESSENTIALS — TIER ONE
Biweekly Wellness assessments + preventive nursing oversight
(2 Nursing Visits Per Month)
This tier gives families a dependable baseline of professional care, early detection, and peace of mind.

Full head-to-toe assessment every visit to catch changes early.

High-touch monitoring and ongoing evaluation.

Nurse attends 1 medical appointment per month (in person or virtual) to support, clarify, and help with medical decisions.
